North American Health Certificate Request

North American Health Certificates are a foundation document for producing and providing the history of a certified seed potato lot. As indicated, all certifying agencies in North America utilize this document. This document is provided to other states/provinces as seed is shipped. The Colorado Seed Act also requires this document for all seed lots sold within or imported into the state of Colorado. Receivers must keep a North American Health Certificate for each lot received on file in case of an audit by the Colorado Department of Agriculture.
